Media Release Form This release form is to notify all attendees of Rare Bootcamp that a team from CNBC (a national broadcast media outlet, also referred to here as “the network”) will be onsite on April 9, 2026 to obtain b-roll footage (background video and audio) for the purposes of a potential future media segment that could air on CNBC’s broadcast, web-based, or podcast platforms. As a result, video, images, or voice recordings (collectively “the footage”) of your participation or presence at the event could be obtained by CNBC while its representatives are onsite. CNBC will have unlimited right to edit release, publish, broadcast, display, use, and post on the Internet the footage it obtains at Rare Bootcamp, either in whole, or in part, in whatever medium the network deems appropriate. This use could include education and awareness campaigns on its CNBC Cures platform; public relations; internal and external training and education; disease awareness campaigns; internal communications; and/or sales and marketing activities (including, but not limited to, websites and social media). Ultragenyx Pharmaceutical Inc. and all other sponsors and agents of Rare Bootcamp will not own or control the footage or derivative works thereof, and are not responsible for its future release and use. By signing below: I acknowledge and agree that CNBC owns the footage and all ownership, copyright, or other rights related to it. I further acknowledge that all rights in the footage, including any copyrights or other proprietary interests of whatever kind, shall belong exclusively to CNBC. I release and discharge Ultragenyx and all other Bootcamp sponsors and agents from any and all claims and demands arising out of or in connection with the use of the footage including, without limitation, any claims for libel or violation of publicity or privacy. I understand I will not have an opportunity to inspect and/or approve the use of the footage, or any printed or electronic matter that may be used with it, or any reproductions thereof by CNBC, and that I may not be informed in advance of the specific use of the footage. I understand that my consent is voluntary, that I am not required to sign this Release Form, and that I may in fact refuse to sign it, thereby prohibiting CNBC from including me in the footage.
